Unlike institutional care, where dignity and self-determination may seem threatened, at-home care offers psychological benefits that can help patients improve and even thrive. With proper medical attention, many problems can be managed or resolved more effectively and less expensively at home. Even in the case of Alzheimer’s disease, providing daily structure can extend the early, less debilitating stage for a longer period of time to keep the patient out of a nursing home. Trimark’s flexible services are designed to meet the full range of at-home healthcare and personal needs- from a few minutes a day or a few hours a week to round-the clock assistance and total care management. Some
patients need temporary assistance as they recover from an illness, fracture
or stroke. Or perhaps a family
caregiver wants time away without worry or guilt. Others need a longer term approach for
managing permanent disabilities or chronic diseases such as diabetes, cancer
or heart disease. More intensive
services and regular monitoring may be required to cope with Alzheimer’s and
other degenerative diseases in the home setting.
When
home is in a retirement community, Trimark’s Shared Services enhance the
programs of the community itself. Choosing
the Level of Assistance You Need During an initial interview, a Trimark care manager will help you determine what assistance is needed, how often, at what cost and whether any insurance or Medicare reimbursement is available. In many cases, patients need only an hour or two of daily assistance to remain safely at home instead of entering a nursing facility. Typically, this involves help with getting out of bed, dressing, taking medications, getting to meals and preparing for bed at night, with periodic monitoring several times throughout the day. In a retirement community with onsite Trimark staff, this level of assistance can be accomplished in less than two hours a day. This is about the same amount of staff contact time a patient receives in a nursing home but at far less cost. As a patient’s needs change, Trimark caregivers adapt the program to provide the most appropriate type and level of support. Throughout the process, we help families make realistic decisions about illness, finances, the final stage of life and other matters of concern. Personal
Care Services for Everyday Living Personal
care services are generally provided on a daily or as needed basis by Trimark
nursing assistants, who are supervised and monitored by an RN case
manager. They can include some or all
of the following: § Assistance
with bathing, dressing, grooming and managing incontinence § Meal
preparation or delivering meals from a community dining room § Assistance
with getting to and from the dining room § Light
housekeeping and laundry § Security
checks and monitoring, ie., taking patients to the bathroom, adjusting the
thermostat, fetching needed items and other periodic tasks § “Nurse
Call” service activated with a 24-hour emergency call button §
Medication set-ups and reminders § Transportation
to and from doctors’ appointments and activities §
Assistance with exercise routines §
Companionship Individualized services for special needs can also be arranged. Our flexible approach allows you to schedule the appropriate amount of care needed as health conditions change.
Case
Management Simplifies Total Care Complex

A Trimark case manager- an RN experienced in geriatric care – is in
the best position to see the total picture as she coordinates with physicians
and other home health care providers, manages medications and structures a
“get well” program aimed at maximizing medical care effectiveness. When
accompanied by a program of personal care assistance, most Trimark case
management services are provided at no additional cost. Your case manager can supply as much
assistance as you desire, including full decision-making responsibility as a
surrogate family member. Trimark

and evaluating the patient’s health status during regular visits and whenever
problems arise § Teaming
with physicians, social workers, Medicare home health care providers,
hospitals and others to coordinate and carry out a total care plan § Consulting
with family members on care needs § Performing

emergency visits at any hour of the day or night § Making
referrals to community and governmental agencies when appropriate § Supplementing
home hospice programs An extension of the family, the case manager is the patient’s advocate. Her professional status gives her timely access to medical, community and institutional resources not as readily accessible to family members. As a knowledgeable facilitator, she communicates and interprets vital information between the medical world and the patient, easing fears and assuring the best possible treatment program.

Services Reduce Costs In
retirement communities served by Trimark, residents share the services of
on-site nursing assistants. This
enables them to receive low-cost assistance for short intervals several times
a day, if necessary. As many as a quarter of a community’s residents use Trimark Shared Services for one-half hour or more per day to improve their health status and remain independent. Flexible arrangements allow any combination of shared and private duty service as patient needs change.
Supervised by an RN case manager, on-site Trimark nursing assistants provide whatever daily or weekly personal assistance is needed to help residents live safely and comfortably in their own homes. Many of our retirement community caregivers are permanently based, allowing them to develop personal, on-going relationships with their patients. Private Duty
Options If extensive, live-in or continuous care becomes necessary, Trimark can provide private duty nursing and custodial assistance in the patient’s home for short or extended periods of time. In addition to the psychological benefits of home-based care, patients receive much more attention and personal contact than they would in a nursing home. Trimark nursing assistants are assigned on a continuing basis to promote meaningful companionship and a greater feeling of security. Financial
Arrangements The cost of Trimark’s services is determined by the total hours of care provided. To keep costs as low as possible, we work with you to coordinate service with Medicare providers, making referrals when appropriate. Some private insurance policies will cover home care.

care provider. Charges At
the beginning of service, Trimark’s case manager works with the patient and
family to determine the services that will best fulfill the patient’s
needs. Adjustments are made in
services according to the needs of the patient over time. Reduction in care and cost are made when
the patient’s condition improves.
Charges accumulate based on the total time spent with the
patient. There are no extra charges
for nights or weekends. Specified
holidays are billed at time and a half.
